to Postal Plus in person. An application must be completed for each person requesting
Postal Plus to provide mail services. This form is a very important legal document which must be properly
completed before commo carrier package/parcel services and US mail service may commence. It will be
maintained by Postal Plus and the US Post Office, Bryan, TX. You may print (legibly/blue or black ink)
or type this form.
